Non-Vascular IVR
What Is Non-Vascular IVR?
Non-vascular IVR refers to a category of IVR (Interventional Radiology: image-guided therapy) in which, rather than accessing the body through blood vessels, a needle or device is inserted directly through the skin to perform treatment or examination of a lesion.
By confirming the position using imaging such as CT or ultrasound, it is possible to precisely target the lesion and perform highly accurate, minimally invasive treatment.
The main procedures include:
- Radiofrequency ablation (RFA)
- Percutaneous drainage
- Biopsy (tissue sampling)
- PVP (percutaneous vertebroplasty)
- Pain relief treatment and nerve block
- Stent placement for the trachea or digestive tract
- Gold marker placement
These are just some of the procedures we offer.

Main Treatments and Procedures
Radiofrequency Ablation (RFA)
Radiofrequency ablation (RFA) is a treatment in which a fine needle is inserted into a lesion and the tumor is necrotized using heat.
The main targets are:
- Lung tumors
- Liver tumors
- Renal tumors
- Metastatic tumors
- Bone metastases
- Lymph node metastases
These, among others, are treated with the aim of local control.
No skin incision is required, treatment can often be completed on a short-stay or same-day basis, and the ability to repeat treatment is also a characteristic feature.
At YASUMOTO IVR Clinic, the director—who has extensive treatment experience—evaluates indications based on detailed image analysis and performs precise local treatment under CT fluoroscopy. We also handle advanced cancer treatment combining vascular IVR with RFA (TAE combined with RFA).
Percutaneous Biopsy (Tissue Sampling)
This is an examination in which a needle is safely inserted into a lesion under imaging guidance to collect tissue.
It plays an important role in accurate diagnosis and determination of a treatment plan. We primarily perform lung biopsy, lymph node biopsy, and bone biopsy, mainly on a same-day basis.
Percutaneous Drainage
This treatment is performed when pus, bile, urine, or other fluids have accumulated inside the body due to infection or blockage of a passageway within an organ.
Under imaging guidance, a fine tube (catheter) is inserted through the skin to drain the accumulated fluid to the outside of the body.
Main conditions treated include:
- Abscess
- Pleural effusion, ascites, lymphatic fluid accumulation
- Biliary obstruction
- Urinary tract obstruction
- Postoperative complications
and others.
It is an important treatment for symptom relief and stabilization of general condition.
PVP (Percutaneous Vertebroplasty)
This is a treatment for compression fractures of the vertebra caused by osteoporosis or metastases.
A needle is inserted under imaging guidance, and bone cement is injected into the vertebra to stabilize it, with the aim of achieving:
- Pain relief
- Early mobilization
- Improvement in QOL (quality of life)
among other benefits.
Pain improvement may be observed as early as the day of treatment.
Pain Relief Treatment and Nerve Block
This treatment is performed for pain caused by compression or irritation of nerves by tumors.
Under imaging guidance, medication is administered around the nerve to relieve pain.
At our clinic, we primarily perform celiac plexus nerve block and similar procedures to alleviate cancer-related pain.
Stent Placement
When the trachea or digestive tract has narrowed or become blocked, a metal tube (stent) is placed to secure a passage for air or food.
This is performed with the aim of improving daily life functions such as breathing and eating.
High-Precision Treatment Using CT Fluoroscopy
Features of CT Fluoroscopy-Guided Puncture
In non-vascular IVR, the accuracy with which the lesion’s position can be confirmed during treatment is closely related to safety and treatment precision.
At YASUMOTO IVR Clinic, we actively perform CT fluoroscopy-guided puncture.
With CT fluoroscopy:
- The position of even deep-seated lesions can be accurately identified
- It is less affected by gas and bone
- The three-dimensional spatial relationship with surrounding organs can be assessed
These features enable safer and more precise puncture treatment.
In particular, at our clinic we use a CT fluoroscopy system with simultaneous 6-screen display, providing precision puncture treatment by IVR specialists.
Selective Use of Ultrasound
Ultrasound has the advantages of no radiation exposure and allowing simple real-time observation.
On the other hand,
- Deep-seated lesions
- Lesions affected by intestinal gas
- Lesions near bone
- Areas containing air, such as the lung
may be difficult to visualize with ultrasound.
At YASUMOTO IVR Clinic, we appropriately select between CT and ultrasound according to the location and characteristics of the lesion, with a focus on safety.
Commitment to Radiation Protection
X-ray Protection Board "kamakura"
To reduce radiation exposure during CT fluoroscopy procedures, YASUMOTO IVR Clinic has introduced the radiation protection board “kamakura,” a device developed with the involvement of our director.
By reducing scatter radiation exposure while maintaining ease of puncture operation, we work to establish a safe IVR treatment environment.
When the operator can perform precise treatment in a stable environment, this ultimately also contributes to improved safety for patients.

Academic Background and Our Clinic’s Role
In non-vascular IVR, it is important to accurately confirm the position of lesions using imaging and perform puncture and treatment safely.
Our clinic participates as the secretariat of the Puncture and Drainage Society of Japan, contributing to academic activities and technical exchange related to image-guided puncture treatment. We are also engaged in the advancement of IVR medicine through technical instruction for younger physicians and research activities.
Features of Our Clinic
At YASUMOTO IVR Clinic, we offer:
- Precise treatment using CT fluoroscopy-guided puncture
